About the NCMA Live Oak Family Medicine team

A patient-centered medical home

NCMA Live Oak Family Medicine (formerly Spring Creek Family Practice) was founded in 1970 and run as a solo practice for decades. The practice first partnered with Northern California Medical Associates in 2008. Voted 2011 Best Family Doctors in Sonoma County in Northbay Biz magazine, our board-certified physicians James G. Trapnell, MD, and William A. Markoff, MD, and Nurse Practitioner Jennifer E. Warren, FNP, believe that getting to know their patients and understanding their personal lifestyle goals are the first steps towards providing quality, personalized healthcare.

At NCMA Live Oak Family Medicine, physicians and nurse practitioners understand how making healthy lifestyle choices isn’t always easy, but the benefits of leading a healthy life are within everyone’s reach. For more than a decade, Drs. Markoff and Trapnell have had the pleasure of making Sonoma County residents healthier and happier in their day-to-day lives.

Office-based prevention and screening programs include general and sports physical exams, routine check-ups, vaccinations, laboratory testing, behavioral healthcare, women’s health, among other primary medical care services. Early screening and detection can result in less acute episodes of illness that are devastating both physically and financially. NCMA Live Oak Family Medicine physicians provide primary healthcare at every stage of life.

William A. Markoff, MD

William Markoff, MDWilliam Markoff, MD, grew up in Santa Rosa, Calif. and graduated from Loyola University of Chicago’s Post-Baccalaureate Pre-Medical Program in 1993. He attained his medical degree from State University of New York Syracuse in 1998. He went on to complete his residency program in family practice at St. Francis Hospital and Health Centers in Indianapolis, Ind., during which he also worked as an urgent care physician. After returning home to Sonoma County, Dr. Markoff joined NCMA Live Oak Family Medicine in 2001.

On a personal note, Dr. Markoff and his wife are raising two young daughters. He enjoys cooking, drawing, movies, all things electronic, he is a sports enthusiast and is an avid musician and guitarist, but above all, he enjoys spending time with his family.

James Trapnell, MD, Northern California Medical Associates

James G. Trapnell, MD

James Trapnell, MD, was born and raised in the Bay Area. A UC Berkeley undergraduate, class of 1986, Dr. Trapnell went on to attend medical school at UCLA, and then his family practice residency in Martinez, Calif. through UC Davis. Upon completing his residency, Dr. Trapnell worked in the East Bay, California, and in New Hampshire providing healthcare to the underserved. He returned to the west coast in 1999, and has been a family physician at NCMA Live Oak Family Medicine for more than 13 years.
Dr. Trapnell’s enjoys all aspects of family healthcare, especially providing a stable continuum of care for the elderly and medically complicated patients.

Outside of work, Dr. Trapnell and his wife Annette are busy raising their two wonderful daughters. His hobbies include listening to music, cooking, baking, exercise and gardening. Santa Rosa is Dr. Trapnell’s home, and he feels truly fortunate to have become a part of such a wonderful community — both personally and professionally.

Jennifer E. Warren, RN, MSN, FNPJennifer E. Warren RN, MSN, FNP

Jennifer Warren, RN, holds a masters degree in nursing from Sonoma State University and is a Certified Nurse Practitioner and a Board Certified Family Nurse Practitioner. She received a Bachelor of Science degree in nursing from California State University, East Bay.

Jennifer specializes in providing clinical excellence to patients through caring and compassionate service as a family nurse practitioner.

Are you taking new patients?

Do you offer telehealth / telemedicine appointments?

Can I get a flu shot or other injection during my scheduled visit?

Yes. Patients who have a scheduled appointment with their NCMA doctor may ask to receive a flu shot, other injection, or other ancillary service at the time of that appointment.

Are visitors or companions allowed on medical visits?

We ask that patients do not bring any visitors with them to their appointment unless it is medically necessary (ie: a parent of a minor patient not capable of going into an exam solo; a caregiver of patient who is unable to answer questions about health, etc.).

Any visitors who are required for care must also answer all pre-screening questions by phone prior to the appointment. We have limited seating to allow for social distancing. Additionally, our exam rooms are small and it is impossible to social distance in exam rooms with more than 2 people. We ask for everyone’s patience and consideration at this time as we are doing our best to make this a safe environment for all our patients and staff.

Are masks required?

We ask all patients to only enter the building with a mask that does not have a vent. All persons should keep their mask over the nose and mouth for the duration of your visit.

Do I need to pre-screen via phone before entering your office?

Yes. Anyone entering the building needs to be pre-screened by phone first.

We ask that anyone with symptoms of cough, fever, chills, sore throat, runny nose, feeling achy, shortness of breath, nausea or vomiting, new or unusual headache, diarrhea, loss of taste of smell, tingling or numbness, lesions on their toes, or any know exposure to someone with a positive COVID-19 test NOT enter the building.

Can you take walk-in patients?

Due to current health conditions we have stopped all walk-in services. We ask all patients & companions to call prior to dropping by. This applies to injections, EKGs, drop-offs (paperwork or specimens), flu shots and more services.

Patients needing an injection or other service provided by a medical assistant but does not require a visit with a physician, must pre-schedule an appointment for a medical assistant service. These services will be provided Monday 8:00–11:30 a.m., Thursday 8:00–11:30 a.m. & 1–4 p.m., and Friday 8:00–11:30 a.m.
