NCMA diabetes class information

If you’ve been diagnosed with diabetes or pre-diabetes, Northern California Medical Associates’ Diabetes Center is here to help you take control of your health.

Our diabetes self-management class is available for patients with type 2 diabetes. It is ideal for patients who are newly diagnosed or need a refresher and haven’t completed a qualifying class in the past. It works much like a support group. The program consists of four classes of approximately 2–2 ½ hours each, for a total of 9 hours.

Most commercial insurance plans will cover the costs for the course. (Check with your health insurer to determine your actual coverage.) This class also can qualify for your once-in-a-lifetime Medicare benefit.

To take part in our diabetes self-management class, you will need a referral from your primary care physician, who will use our diabetes education referral form. Once we have the referral we will call you to enroll you in a class that fits your schedule.

Diabetes self-management classes take place at our office location at 1701 Fourth Street in Santa Rosa. Each class can accommodate five patients. Class participant patients can also bring one support person, such as a caregiver or partner/spouse.
